Prenatal Care Coordination Services (PNCC) is early entrance and continued participation in prenatal and postpartum care to reduce low birthweight, preterm labor, and infant mortality. Our C.A.R.E. Coaches provide supportive services and referrals to health and human services for pregnant women and their families at risk for poor pregnancy outcomes due to physical, social, psychological, financial, or environmental factors.
Child Care CoordinationServices (CCC) is designed to promote positive parenting, improve child health outcomes, and assistance in preventing child abuse and neglect. CCC will begin assisting the parent 60 days after having the baby and continue (if necessary) until the child's 7th birthday.

Community Resources & C.A.R.E. Packages are available to our Clients to assist with preparing for the birth of the baby and assistance with the on-going care for Clients when they are in need. Resources may include: baby items, housing resources, parenting, employment preparation, etc.
